Services: A medium sized hospice program serving Dallas, Ft. Worth, Houston, and surrounding areas.  Each team consists of at least one full time chaplain, a social worker, nurses, home service aids, and volunteers and support personnel.  Other members of the interdisciplinary team include physicians, pharmacists, social workers, and bereavement counselors.  Works closely with the Parkland Hospital system and a home for persons with AIDS.  Inpatient hospice care available.

Services: End of life care and support for patients and their families in their own homes. Teams of caregivers consist of registered nurses, physicians, home healthcare aides, social workers, chaplains, bereavement counselors and trained volunteers. They provide palliative care to the patient, that is, care that helps ease pain and discomforting symptoms. In addition, they provide psycho-social and spiritual support for both patients and their loved ones.

Services: The Palliative Care team focuses on physical, psychosocial, spiritual and emotional care and comfort. Bereavement care is also provided for caregivers for a least one year, which includes memorial services four times a year.

Hours:  The office is open 8:00 am - 5:00 pm Monday through Friday.  Nurses are available and on call 24 hours a day, 7 days a week.

Languages:  The Visiting Nurse Association uses a translating phone service, so all languages are available.

How to Apply:  Referrals need to be made from a doctor to the Visiting Nurse Association for home care.  For hospice services, the VNA needs a doctor's diagnosis that a patient has six months or less to live.  The VNA will then evaluate the patient and place them on services if approved.

Cost of services:  Cost depends on each individual case.  The VNA accepts Medicare and Medicaid, as well as some private insurance companies.  Scholarship money raised at fundraising events may be available to patients in need.

Other information:  A bereavement program is offered to patients and family to help them cope with the grief of loss.  Chaplains, Social Workers, and volunteers lead classes and workshops.  Safe Haven is a bereavement program for children under 18 who are dealing with the loss of a loved one.  These classes and workshops are available throughout the year.
